Our systems will store the details of all medicines and products that we have supplied to you on either an acute or repeat basis.

Your right to view your health record You have the right to ask for a copy of all the information we store about you. Your request must be made in writing to our postal address listed in our Terms and Conditions and you must provide enough identification for our staff to identify you. A copy of your passport, full driving license, and/or credit/debit card will be required before information is released. We will respond within 40 days of receipt of documents. Please note there will be a charge of £15 to have a printed copy of the information held about you and to cover our overheads and postage costs.
